Requests for authorization from vendors outside Open Application Period 5 will only be considered in areas where there is inadequate participant access as defined by New York State. The Zip Code List identifies areas where stores are needed. If your zip code is on this list, complete the “NYS Request for Application” and submit the document, along with all required attachments to Public Health Solutions at vma@healthsolutions.org.
